During the first quarter of 2026, Sandpiper Post Acute ranked as the largest nursing home facility in Charleston County, sharing this position with White Oak Manor – Charleston, data from the Centers for Medicare and Medicaid Services (CMS) shows.

Records indicate the facility provided 176 available beds, which is 63 more than the Charleston County average in early 2026. It served an average of 156.2 residents within that time frame.

The CMS gave Sandpiper Post Acute an overall rating of 1 out of 5, notably lower than the South Carolina nursing home average rating of 3.

By the end of the first quarter of 2026, Sandpiper Post Acute had paid $84,974 as a result of five fines and faced five separate penalties.

Charleston County was home to nine for-profit nursing facilities overall.

U.S. Census Bureau estimates indicate that the population of older adults nationwide continues to increase while younger demographics decline. From 2020 to 2024, Americans aged 65 and up grew in number by 13%, whereas the under-18 population decreased by 1.7%. Over the past 100 years, the older adult segment has expanded rapidly, rising from 4.9 million in 1920 to more than 61 million in 2024.

A 2021 report found that about half of adults in this age range, equaling roughly 28 million people, could eventually require long-term services and supports. The same analysis projects that above a third may someday need to reside in a nursing home.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
